Patient Support Program
Biodesix is committed to making VeriStrat accessible to patients who need the test. As such Biodesix has established the Financial Assistance Program (FAP) to help reduce or eliminate cost a patient might owe. Qualification for the program is based on the patients individual financial need.
To apply for the financial assistance program the patient must submit a completed application. Once the application is received, Biodesix will review the application and determine eligibility.
Please note that a Biodesix representative will be appointed to each patient who seeks FAP assistance. This representative will offer assistance with various tasks, such as completing and submitting the application, appealing a denied application, and establishing a payment plan if an appeal is denied.
Eligibility Requirements:
If patient is uninsured and has no medical coverage the following criteria apply:
1. The patient must not be eligible for medical coverage through any local, state or federal programs.
2. The physician or facility must be providing no-cost or reduced-cost treatment for the patient.
If the patient has private insurance the following criteria apply:
1. The patient’s annual adjusted gross income must not exceed $150,000.
2. The patient must be able to provide acceptable proof-of-income documentation.
3. The patient’s net worth cannot exceed $2,000,000.

If I have private insurance, what is the cost for VeriStrat?
If you have private insurance, coverage is dependent on your specific plan. Depending on your specific insurance plan, you may be responsible for aco-payment, co-insurance or deductible. To helpyou determine your maximum financial liability, Biodesix provides the VeriStrat Support Hotlineat 866-432-5930. A customer service representativeis available to answer your questions.
Biodesix will file your insurance claim for you. This process can take time and you may receive an insurance statement that looks like a bill. These statements are often called EOBs (explanation of benefits) and are not bills. If you receive any insurance forms that you have questions about, please call the VeriStrat Support Hotline at866-432-5930. A customer service representative will help you with any questions or concerns.
What if I can’t afford my part of the cost for VeriStrat and need financial assistance?

If my insurance is through Medicare, how much will VeriStrat cost me?
Similar to other insurance providers, we will file your claim for you. Biodesix anticipates that Medicare will pay for the cost of VeriStrat testing. Since Biodesix is not collecting an Advanced Beneficiary Notice (ABN) from Medicare patients, Biodesix cannot hold you financially responsible for any costs that are not covered by Medicare. The claims process with Medicare can take a long time. If you receive any notices or correspondence regarding the test from Medicare, please call the VeriStrat Support Hotline at 866-432-5930 for assistance.
What if I don’t have insurance? Can I still get the VeriStrat test?
For patients who do not have insurance and are in financial need, the Biodesix Financial Assistance program can help. For qualified patients based upon individual financial need, the test will be provided at no cost.
