How to order a VeriStrat® test
Step 1: Order a Serum Collection Kit
Obtain a VeriStrat Serum Collection Kit if you do not already have one. Orders are shipped in a box containing 3 Serum Collection Kits.
Step 2: Complete the Test Request Form
Complete the VeriStrat Test Request Form. Make sure all fields are filled in and the form is signed by the ordering physician.
Step 3: Collect the sample
Collect a sample from your patient by following the serum collection instructions.
Step 4: Ship the sample
Prepare the sample for shipment by following the shipping instructions.
Step 5: Receive Results
Results are provided typically within 72 hours of sample shipment.
