What to Include
Materials Required for Shipping Sample:
1. VeriStrat® Sample Collection Kit, which includes the following:
- Shipping container (EPS foam cooler and corrugated outer box) marked with Dry Ice Decal, Orange Arrow Stickers and Exempt Human Specimen Label
- FedEx® Return Air Bill
- Bubble Wrap, Specimen Pouch (Biohazard bag with absorbent sheet), and Document Pouch
2. 2.5 kg dry ice (not provided)
Instructions for Shipping Prepared Samples:
In the event a sample cannot be shipped immediately, sample must be stored frozen in a -80o freezer (preferable) or on dry ice. Sample must be shipped within one week of the sample draw date.
