Shipping Instructions

In the event a sample cannot be shipped immediately, sample must be stored frozen in a -80°C freezer (preferable) or on dry ice.  Sample must be shipped within one week of the sample draw date. Please note that Biodesix will not receive samples on the day of observance of the following holidays.

1. Place the cryovial with the patient identification label, containing 0.1 mL of serum, into the biohazard specimen bag with absorbent sheet. Seal the specimen bag.

2. Wrap the sealed specimen bag using the supplied bubble-wrap sheet, place in the VeriStrat® shipping container, and cover with dry ice. Please ensure that there are 5 lbs (2.5 kg) of dry ice in the shipping container at time of shipment.

3. Insert the top copy of the completed VeriStrat Test Request Form in the accompanying document pouch (10″ x 10″ plastic zip-seal bag). Retain bottom copy for your records. NOTE: SPECIMENS SHIPPED WITHOUT A TEST REQUEST FORM WILL NOT BE PROCESSED.

4. Place the document pouch on top of the dry ice, inside the VeriStrat shipping container.

5. Place the styrofoam lid on top of the VeriStrat shipping container, close the outside box, and
tape securely.

6. Fill in your name and address on the preprinted FedEx® air bill. Check the dry ice box under section 6 and record the weight. Retain the top copy of the completed preprinted FedEx air bill for your records and tracking. Place the completed preprinted FedEx air bill in the domestic air bill pouch. Apply the pouch to the outside of the VeriStrat shipping container.

7. Locate the dry ice shipping decal on the outside of the VeriStrat shipping container and fill in the dry ice weight (kg).  Also complete your name and address in the “shipper’s name” section.

8. If your facility does not have standard FedEx pickup, arrange for immediate pickup by calling 1-800-GO-FEDEX (1-800-463-3339).

9. Confirm timely pickup of sample.
