Materials Required Prior to Collecting Patient Sample:
1. Completed and signed VeriStrat® Test Request Form
2. VeriStrat Sample Collection Kit, which includes the following:
- SST Vacutainer
- Transfer pipette
- Cryovial
- 2 sample identification labels
3. Clinical centrifuge (not provided)
4. Dry ice (not provided)
